Please send a copy of the attached Medical Authorization Form
with your child and their care provider/chaperone.
This form should accompany any minor competitor that will be attending the 2011 Two Rivers Féile with a care provider and/or
chaperone other than their own parent and/or guardian.
Please Note: As stated in article 39 of the 2011 Two Rivers Féile syllabus--
Medical treatment will not be provided to any minor competitor
without the sole written consent of both of their parent(s) and/or guardian(s).
